MENTORING PROGRAM PARENT/GUARDIAN PERMISSION FORM

The mentors to this program have been carefully screened and trained. Young people and mentors are prohibited from meeting face-to-face, on the telephone, or at any other place on the Internet without your express permission. The program will last for at least a year, requiring 4 hours engagement per month.
To help us measure and evaluate the benefits and effectiveness of the program, your child will be required to furnish the office with periodic information through survey
